| Instructions for Filing DEP Forms | These instructions & guidelines apply to the following DEP Forms:
WPA Form 1 Request for Determination
of Applicability
WPA Form 3 Notice of Intent
WPA Form 4 Abbreviated Notice of
Intent
WPA Form 4A. Abbreviated Notice
of Resource Delineation

| Application Requirements | All applications must be completed (1 original application with 7 copies)with payment for filing fees,and recieved by the Westfield Conservation Commission no later than 11:00 AM 8 business days prior to the desired Conservation Commission Meeting.
The Westfield Conservation Commission meets every second and fourth Tuesday of each month.(Excluding Major Holidays & Summer Schedules) Your meeting will be posted by the Conservation Commission in the Westfield Evening News paper. The Westfield Evening News will bill the applicant directly. |
| My Project is Complete What's Next | If you have filed a Notice of Intent or an Abbreviated Notice of Intent and your project was issued an Order of Conditions (WPA Form 5); once all conditions have been met you will need to submit a Request for Certificate of Compliance (WPA Form 8A).
If the project received an Order of Conditions from the Conservation Commission, submit the request to that Commisson. If the project was issued a Superseding Order of Conditions or was subject of an Adjudicatory Hearing Final Decision, submit the request to the appropriate DEP Regional Office. |
