Where do I go for medical attention?

Noble Hospital is our preferred provider. We have arrangements with them to treat our work-injured employees. Just identify yourself as a City employee and that you have received a work-related injury.
For follow up care, Noble Hospital Occupational Health Clinic is our preferred provider, and is available to take care of you.
Should you choose to receive follow up medical care from another provider, you are responsible for having a “Physician’s Medical” form completed by your doctor and returned to the City for each visit. The forms are available from your department and/or the Personnel Department at City Hall or the Benefits Office for the School Department at Ashley Street.
