Types of Certificates:
Birth: The Westfield City Clerk's Office issues certified copies of birth records for a child who was born in Westfield or whose parents resided in Westfield at the time of the child's birth.
Death: The Westfield City Clerk's Office issues certified copies of death records of any person who died in Westfield or if the person was a resident of Westfield at the time of their death and died in another community in Massachusetts.
Marriage: The marriage record is registered in the City where you filled out your "Marriage Intention" paperwork.
*Some birth and marriage records are not public documents and a picture ID is required in order to obtain the record. Therefore, this type of transaction cannot be processed electronically. You can call the City Clerk’s office at (413) 572-6258, to check on a record at any time.

Certificate and License request forms can be filled out and returned with a check and self addressed stamped envelope, to the City Clerk's office to be fulfilled.
